Responses to Solicitations submitted through this website for Patch requests must be made via mail to the department address: 3101 North State Road 7 Hollywood, FL 33021 with attention to our Logistics Department.  Return addresses will not be honored to Post Office Boxes and must contain a street address. We may verify all requests and reserve the right to decline any request without explanation. All requests will be made in accordance to the applicable law.
